The red colored element (נרכש ב 30 יום אחרונים) indicates it has gone missing. How that came to be, is another question altogether - probably some failed rename/move of a model/package (I see that the package named 2 includes a Label with the same name). If you can recreate this somehow we would welcome the details.
Validating the project you provided as is, initially shows no errors, because the configuration file defines the root system as SmartNotes4/SmartNotes4, which is missing (this might be considered a bug in its own right ...).
Changing the root system to QHErr/QHErr and re-validating, reveals a few errors, including the missing model, which should be fixed.
